Step 1

Fill out and submit the online activation form.

Step 2

A Monarch Well Works team member will contact you by email with an activation code.

Step 3

Follow the email instructions then enter your new user name and password.

Step 4

You’re all set! You can access your patient portal at any time. Sign-In

YourHealthFile® is a secure connection between you and Monarch Well Works.

Use the patient portal to schedule appointments, communicate with your care providers, see results and so much more! You must first activate your account before signing into YourHealthFile®.
